The progression of low-grade B cell lymphomas into aggressive lymphomas is a well-described phenomenon in CLL/SLL (Richter syndrome) and follicular lymphoma, most commonly into DLBCL. In contrast, transformations in marginal zone lymphoma are less well characterized.  While plasmacytic differentiation is common in the cytologic spectrum of the disease, overt plasmablastic transformation is rare. We report such a case of marginal zone lymphoma, presenting initially in its classic form, later mimicking a plasmacytoma, and eventually undergoing aggressive transformation to plasmablastic lymphoma.
