Iliac artery endofibriosis is a rare, benign condition observed usually in competitive cyclists, presenting with symptoms of lower limb ischaemia. Psoas muscle hypertrophy in these athletes is thought to irritate the external iliac artery where they cross, resulting in intimal thickening of the artery and subsequently narrowing the artery. This condition is often identified only using imaging techniques, and managed conservatively.

This condition is often identified only using imaging techniques, and managed conservatively.
Histological sections of this condition are exceptionally rare, and to our knowledge the only case in our laboratory.
We present a case of an athlete, along with their imaging and finally the histology.
